Apply to get support for our rehabilitation center outpatient services or to be paired with a service dog.

Our service dogs work incredibly hard to ensure their veteran remain actively outdoors and constantly happy for years to come. Each service dog is placed to perform a specialized task tailored to their veteran. Unlike other organizations, there is a minimum standard we establish to ensure that our service dogs and veterans thrive together.

SUPPORT APPLICATION

A short essay (minimum 750-words) is required in order to apply for our programs. You understand that by submitting this form along with an emailed essay, you agree to our Terms & Conditions, you are of legal age of consent, and you are mentally capable of carrying out the terms of this agreement. After completing this form, please email us an essay telling us about yourself, your military service, and what support you need. Essay must be in MLA Format with size 12 (Times New Roman) font. Email your essay to info@warriorschoice.org.

AMPUTEE RESOURCES

The Amputee OT resource was created with help of amputees and healthcare professionals that work with amputees to support the amputee community after they stop receiving formal therapies. Click on the link below to access resources on non-opioid pain management techniques, accessing resources, support groups and much more! Remington is ready to be paired with a handler to help them lead a better life.

The Warriors Choice Foundation provides access to the incomparable Focused Cognitive K9 Rehabilitation (FCKR) program, which since 2018 has fast-tracked the process for those needing a service dog.

This program provides an in-house breeding & training program, and a remote program for transitioning personal pet dogs into working service dogs.
